Listed below are some of the 4-H forms used by 4-H club leaders and club officers:

Each year, clubs must complete the 4-H Annual Charter Renewal and submit them to the UW-Extension Office by December 1.

The 4-H program encourages members to use basic parliamentary procedures for their club business meetings. The following brochure defines parliamentary procedures and the steps in making club decisions.
