4-H Afterschool at CFC



4-H Afterschool Program Registration Form

Students in Grades K-6 in 2017-2018 school year

Monday – Friday          

3:00 p.m. – 6:00 p.m.  After School

COST: $5 per student per day


Students receive a healthy snack provided by 4-H Afterschool. 4-H Afterschool staff is youth protection certified and experienced, trained college and high school students.

4-H Afterschool aims to provide a safe and secure place for youth to share fellowship, recreation, service and learning. This means that youth in 4-H Afterschool’s program will participate in hands-on learning experiences that help them develop life skills and foster caring relationships with adults and peers while having fun in a safe, welcoming environment.

How to Register Online

The 4-H Afterschool program registration will be entirely online in 2017. In order to register you need to:

  1. Go to the following Google Form link:  https://goo.gl/forms/f68gsxkxfCMRXklH2
  2. Complete the Registration Form online. Please submit one form per family.
  3. On or near August 25, 2017 you will receive email confirmation of registration with more instructions about how to enroll or re-enroll your child utilizing 4HOnline by September 5, 2017.
